Where I can find the Form I regarding Fly ash disposal

Can anyone guide me where i can find the form I for the ash disposal OR New form in which the fly ash disposal details are submitted to MOEF every year. Also the latest notification regarding Fly ash Utilization.

I think it should be in HW(M&H)Rules,08 and its subsequent revision.

There is another notification of FA published in 2008

I think flyash disposal is an integral part of coal burning, and must form part of the application for thermal power project, and must be submitted in the recommended form for thermal power projects.
